it can depend a lot upon what type of plant it is, but generally if you disturb a plant you want to balance out the amount of roots with how much green stuff there is up top. so knowing when you disturb a plant that you'll be losing some roots to the disturbance then you can probably help...

most of the time in the past for me it has been the strawberries that have done that.

some brightly colored rope tied to the handle may help find it. Mom also often loses hers but they are usually just dumped on the weed pile out back because she put them in the weed bucket. if she tells me where she's been i can often find them for her if they're not in the weed pile.
