A&L Eastern does:

Organic Matter
Estimated Nitrogen Release,
Available Phosphorus,
Exchangeable Potassium,
Magnesium,
Calcium,
Hydrogen,
Soil pH,
Buffer Index,
C E C and Percent BaseSaturation of Cation Elements,
Sulfur,
Zinc,
Manganese,
Iron,
Copper,
Boron
Plus they give fertility recommendations (I take those with a grain of salt since I know my soils history better than the lab). The total cost for the test is $14.40 unless you want the Bray for P and ammonium acetate for the cations, then it's $16.30. I prefer the Bray so I go with the more expensive test.
