I have a fertilizer that mixes 1 tsp per gallon and it is ready to use. What I want to do is make a concentrated liquid solution where 5ml equals 1 tsp per gallon and I have a 250 ml mixing bottle for the concentrate

1 teaspoon essentially equals 5 ML. So fill your 250 ML measuring bottle and add 5 ML to a gallon of water.

Most fertilizer and such things tell you to use rather quickly after dilution. Make sure that is non the case.
